Cal Stage Faxable/Mailable Forms

Many types of equipment rentals (truss, rigging gear, other potentially hazardous items) require the customer's signature on a Cal Stage Liability Waiver. Essentially, this asserts that the customer has the necessary knowledge/ability to use the equipment to be rented.
Some other sorts of equipment have possible dangers of their own, and for those, we offer these:
Many types of equipment rentals (truss, rigging gear, moving fixtures, large packages or expensive packages) will require insurance on the gear, and liability insurance for the event. Full details of what is required can be seen on this Sample Insurance Form. If you have specific questions that are not addressed by the sample (such as the value of the gear on your rental) please ask your Rental Consultant - they can provide this information.
For Credit card purchases being made by other than the cardholder, or use of a Credit Card for rentals of equipment or as a "security deposit" (Rentals or Sales), Cal Stage must have a signed, completed copy of the Cal Stage Credit Card Authorization form on file. Please note as of 9/22/07, Cal Stage's Credit Card form has changed, and we now require, in addition to your completed, signed form, a xerox of your Driver's License, and a xerox of the front and back of the Credit Card you wish to use. Cal Stage cannot accept Debit Cards for security against a rental; it must be an actual Credit Card. We must receive all three items (Credit Card Authorization Form, xerox of Driver's License, xerox(es) of front/back of Credit Card) in order to accept/charge your card. We are sorry for the inconvenience, but it is necessary.
Although Cal Stage is not always in the market for new hires, we are always interested to receive resumes and other material. Please submit a Job Application along with your information.
Companies or Jobbers or other groups or persons who wish to do business with Cal Stage on an account basis, will need to submit a signed and completed copy of the Cal Stage Credit Application form. Please print and fill out both pages where indicated.
Please note: Both credit forms must be completely filled out and signed, or the application cannot be processed; processing of your application will be as timely as possible, but is almost entirely reliant on the timing of the response of your references. Some may require that we mail a request to them, while others may provide a reference with receipt of a faxed request; so please allow a minimum of 4 weeks for processing. You will be notified of our response to your request, but keep in mind that our receipt of your completed paperwork does not guarantee an account.
